Screening questionnaires
Validated questionnaires for ADHD, autism, and functional impact. Use them to help start a conversation with your GP, prepare for an assessment, or track how you're doing over time.
These are screening questionnaires, not diagnostic tools. A positive result means it may be worth speaking to your GP — it does not mean you have ADHD or are autistic. A negative result does not rule either condition out. Always speak to a qualified clinician about your own health.
Autism
AQ-10
Autism Spectrum Quotient
Developed at the University of Cambridge Autism Research Centre and used in NHS settings across Scotland. A score of 6 or more suggests a full autism assessment may be worth pursuing.
CAT-Q
Camouflaging Autistic Traits Questionnaire
Measures how much you camouflage autistic traits — through compensation (social scripting), masking (monitoring your behaviour), and assimilation (fitting in). Particularly useful if you suspect you may be masking at work or in social situations.
Functional impact
These scales measure how much ADHD is affecting daily life — not whether you have it. Useful at every stage: before a GP referral (some NHS boards in Scotland recommend considering WFIRS before referral), at assessment (functional impairment in at least two settings is a diagnostic criterion under NICE NG87), and during treatment to track real-world change.

Weiss Functional Impairment Rating Scale — Self Report
Rate how ADHD affects your daily life across seven domains. Items scored Often or Very often indicate clinically significant difficulty. Useful before a GP referral, at assessment, and during treatment to track change.
WFIRS-P
Weiss Functional Impairment Rating Scale — Parent Report
Rate how your child's ADHD affects their daily life across six domains. Used by CAMHS and private practitioners to monitor treatment outcomes and support referral.
